Pattonville High School uses an online program to house all forms needed to compete MSHSAA-sponsored (Missouri State High School Activities Association) athletics and marching band. Students participating in any athletic team tryouts or marching band, where a physical is necessary, will need to register on Healthy Rosters.
Healthy Rosters is the high school's online site for all forms previously included in the school's participation packet. Physicals, emergency contact, concussion information, insurance information, and all necessary signatures will now be done through Healthy Rosters. The Pattonville activities department can no longer accept participation packets or paper copies of any information.
Parents/guardians will receive an invitation email from Healthy Rosters asking them to register their child. If they do not receive an invitation email or if parents have additional questions, they should contact Alex Hubbs, Pattonville High School's athletic trainer at firstname.lastname@example.org.

Who needs to register?
Parents/guardians of any student who wants to try out for an athletic team at Pattonville High
School. This includes activities such as marching band, drill team and cheerleading.
Why is Pattonville using this program?
This process allows student-athletes' confidential information to be housed in Healthy Rosters and not in Pattonville's files. Pattonville staff will no longer have issues with misplaced forms or coaches needing to know who has a completed packet and who has not. Everything will be online and seen by the families and coaches. Coaches will be assigned their players once they have been turned in a roster. This allows coaches to access emergency information via their smart phones at away games and practices if needed. Healthy Rosters will also allow parents/guardians to see any treatments for injuries their child has undergone. It also gives parents/guardians a direct contact with the trainer for updates.
How will this work?
Parents/guardians will receive an invitation email from Healthy Rosters asking them to register in March. Once a family registers, the student-athlete will go into the system and all forms will be available online for parents/guardians to complete. Incoming freshmen will need a physical dated on or after Jan. 1, 2020 to be eligible to participate in tryouts. Physicals are valid for two years, but a new physical must be completed prior to an eighth grader trying out for a high school activity. The physical form can be printed from Healthy Rosters or the MSHSAA website and taken to the student's doctor to fill out. Once the doctor has filled out the physical and signed and dated it, families can take a photos of the forms and upload them to Healthy Rosters. Pattonville's athletic trainer will then review it and accept it or request more information. Healthy Rosters will give
families an up-to-date checklist of what is needed for participation.

Pattonville's athletic trainer, Alex Hubbs, has his information listed on the forms families will
receive by email. Feel free to contact him with any questions. Hubbs will also be present at
registration with a laptop to help anyone who needs further assistance with Healthy Rosters.
